How to qualify for Laparoscopic Sleeve Gastrectomy alone.
A) Your medical insurance does not cover bariatric surgery
B) Must be a candidate for weight loss Surgery (BMI >30)
C) Must be a surgical candidate for an outpatient procedure (Same day discharge) at the
discretion of the surgeon.
D) Medical, dietary and psychologic evaluation are mandatory for any bariatric procedure
E) This price does not apply for revisional surgery or patients with a previous bariatric
procedure. Such as lap band to sleeve.
Price does not include the following:
A) Initial Bariatric visit with Dr Jorge Acosta and /or Physician Assistant. $200.00.
B) Preoperative Bariatric Visit with Dr Jorge Acosta and /or Physician Assistant. $100.00.
C) 1 week and 2 week Postoperative visit included
D) Subsequent postoperative visits. $100.00
E) Initial Dietician visit $70.00
Subsequent visits at $70.00
F) Initial Psychologist evaluation. $200.00-$300.00
G) Costs of mandatory imaging and laboratory studies if not covered by insurance, and
respective Copays
H) Cost of vitamins, supplements and protein products
I) Mandatory liver reduction diet 2 weeks before surgery
J) Pathology Charge should there be a suspicious lesion in the extracted stomach.
Combo procedure is when a Cash Pay Sleeve will be combined with another procedure that IS
covered by insurance such as:
a). Laparoscopic cholecystectomy (gallbladder surgery)
b). Umbilical hernia repair
c). Laparoscopic hiatal hernia repair
d). Incisional hernia repair
e). Lap Band and port removal

How to qualify for Combo Procedure.
F) You have a medical condition that can be surgically addressed at the time of a cash pay
bariatric procedure such as Gallstones, certain hernias or removal of lap band.
G) Must have medical insurance. Medicare, Medicaid and most private insurance
companies accepted.
H) Your medical insurance does not cover bariatric surgery
I) Price is only valid for bariatric surgery done at the same time of any of the above
mentioned abdominal surgeries. (Gallstones, certain hernias or removal of lap band).
J) Must be a candidate for weight loss Surgery (BMI >30)
K) Must be a surgical candidate for an outpatient procedure (Same day discharge) at the
discretion of the Surgeon.
L) Medical, dietary and psychologic evaluation are mandatory for any bariatric procedure
M) The abdominal procedure (Gallbladder surgery, hernia surgery, umbilical hernia,
incisional hernia, hiatal hernia repair, removal of a lap band) is medically necessary and
approved by your insurance carrier.
N) Your insurance plan must cover the cost of surgery at East El Paso Surgery Center and
the cost of Dr Jorge Acosta performing surgery.
O) The office of Dr Jorge Acosta will verify if your insurance plan covers abdominal surgery
P) You will be responsible for Copays and deductibles required by your insurance to
proceed with abdominal surgery.
Price does not include the following:
K) Initial Bariatric visit with Dr Jorge Acosta and /or Physician Assistant. $200.00.
